Went in for a root canal and crown which was to be100% covered by my insurance.. Provider outsourced me to an endodontist that was not in network, which I was never made aware of by either offices. After out-of-network endodontist billed my insurance and sent me back to dentist for the crown, dentist said I would owe him $1,500 more to have the crown put in. I am now left with a hole in my tooth and no crown.
