I am a cancer patient. Today my doctor's nurse called the Claremont Imaging Center in Pomona, to request a disk of my CT scan. She was told to send a medical records request, she did. She was told because the request form is outdated they could not send the disk and refused to send it, as a result this can affect my medical care. My wish is that some would review these comments and make changes.
