CKD stage five

I have been going to this doctors office with about four years. I return for my annual check up in September 2022 I was told that my creatine for my kidneys was at 4.5 and that I had chronic kidney disease I was told that a year ago when I had come for my annual check up, McCracken was at 1.7 and I should have been referred to a nephrologist a year ago now I have stage five chronic kidney disease and I have to have a kidney transplant. I thought I was in good hands with Dr. Powell but how can you miss a patient need for care. I have not received an apology yet.
