1. On my first visit to this location, the receptionist was rude, abrupt and unprofessional when I asked her to give me a New Patient Form to take home to complete. 2 Office visit was always with a nurse NEVER Dr Le. I've never gone to a doctor's office before where the doctor NEVER spoke privately with a patient. This was unheard of to me. 3. They told me they only had Versed to sedate me, but this medicine gives me a terrible headache. When I asked if there was something else they could give me I was told "It's Versed or nothing." That was also the first time I've ever experienced anything that. 4. The nurse, Julianna, scheduled an injection for what I thought was my sacroiliac joints but when I was on the table, Dr. Le gave me a caudal injection in my tailbone which was extremely painful! This was NOT what I signed up for and had no idea this was going to be done ahead of time. 5. On my follow-up visit, two weeks later, I told Julianna I was under the impress
