Fast Pace Urgent Care, Livingston, TN

I visited Fast Pace Livingston when I sustained a fall and fractured my wrist. The Nurse Practitioner was quite nice, however, she never physically performed an exam. After obtaining my vitals, the radiology tech performed an x-ray on my wrist. When the NP came into the room, she was carrying the document with the reading from the radiologist, at no time did she exam my wrist. My issue is, an exam was documented as performed when it was not, additionally, the radiologist read the film as negative, a follow up visit to the orthopedic provider revealed it was indeed fractured, he even showed me the location and BTW he did complete an exam. When I called to further inquire, the limit to my conversation was with a billing person. This falls under falsification of documentation. I place this here so that others if they have the same issue will know, this is reportable to the OIG.
