Very poor communication between medical groups

SF: Moved from San Macros to San Antonio 6 years ago. Recently, had to change from Embassy group to Northern Hills. Prescription was sent to pharmacy in San Marcos instead of the one I had been using for last 6 years.JS: Same problem. When I was assigned a new doctor, the prescription was sent to a pharmacy I had used when I first signed up at Wellmed, 11 years previously.
