Dr. Anita Hafeez, DO

Dr Hafeez ignored my privacy concerns and only presented care options which would require a family member to access my medical record. I immediately revoked my permission for her to access my medical information, in ANY capacity-which she totally disregarded and continued to access my record on at least 3 occasions after I “fired” her. I used that specific language-“you are fired and do not have my permission to access my record”. A privacy office said she could, technically, still access my record-regardless of my wishes, since she was still assigned as my primary-but Dr Hafeez knowingly disregarding my wishes and not choosing to have a team PA or nurse access my record instead of doing so herself, is, at a minimum, unethical. By accessing my record she made it clear she totally discounted my concerns, making it VERY clear she would access my record when she wanted, for what ever reason she wanted. Just because administratively she COULD doesn’t mean she SHOULD. Unacceptable.
