Very Despicable doctor.
I have been going to this provider for almost three years. Him and his whole office staff is very unprofessional. The doctor and staff laugh at the patients. They thought I was going but I was recuperating after a procedure. They were making fun of me because of the simple fact that I had asked to see if I can get a RX for a power lift chair. The doctor never thought I was fully awake for the sedative but I heard it all.Obviously he was surprised when I spoke up. His face was full of all colors when I confronted him. I do not know what is his philosophy toward medicine and towards treating patients and I question if he does even has one. I thinks he is an expert in every field, which makes him important, but he is not, what he is is a pure narcissist. His practice has clearly no core values at all. Please read the reviews carefully. I have audio on him plus the techs laughing at me while I was under going the procedure which I am going to make them go viral.

Good doctor terrible office staff.
I would like to start by saying dr. Forman is a good doctor. He took the time to listen and answer my questions. He explain everything I needed to know prior to my procedure. Dr Forman did explain that the procedure would or would not work, but I agreed to go forward. I was aware that it could take up to a week for me to notice a difference. A week later, I was in pain. The procedure did not work on me it and it set me back.. I called the office to let them know and they answered then hung up. I waited a few minutes a I called again only to be put on hold for 12 minutes then they hung up. I called back and they again answered and hung up. It took three weeks for them to call me to let me know my appointment was the following day. Something they did on my 2 prior appointments. I told whomever called that I was not seeing Dr. Forman anymore and I explained why. All I got was ok, I'll put that in your chart. Dr. Forman, you are a good dr but your office staff leaves a lot to be desired.

Dr. Forman had a pleasant manner and explained things in detail for the procedure but his office staff, especially Carina, is absolutely terrible. I agreed to a time with the provider then am called the day prior and told my appointment would be 3 hours later than initially set. This screwed my schedule for that day and made it unable to be seen. Then I'm told I can't get in for almost 3 weeks and an appointment wouldn't be set until the day prior. What a moronic way to schedule.

Very rude and condescending behavior toward my Mother today at a scheduled appointment. We will never go back to this supposed Dr again. He changed my Moms medications without discussing it with her and then disappeared on a one month vacation and closed his office. My 84 year old Mom ended up in the emergency room to get medical treatment. Today was his first day back from vacation and he was angry with my Mom for wasting his time and refused to listen to what happened to her while he was out!

Pleasant and personable. Good bedside manor. Seems knowledgable but didn't follow through with his staff on scheduling we discussed. The medical staff is excellent but the office staff is as bad as it can get! Terrible scheduling for everything time and time again. If I didn't need them I would not go back. I will find a new doctor. Problems with paperwork, perscriptions, scheduling and long waits for procedures and office visits.
