I had injections done in my Facet Joints. Dr and his staff made it the most pleasant medical procedure I have had done. They made sure I was comfortable during the whole process. I highly recommend him for pain management.

I have been treated by Dr. Evangelista for over 10 years. He is prompt, listens to your concerns and works with you to find the solution to fixing the problem. He has treated my knee, neck and shoulder. My pain has been greatly diminished or eliminated. I have recommended him to friends and will continue to do so. Overall one of the best Dr.s I have ever had. Absolutely deserves 5 stars!

I have seen Dr E for back pain, neck pain and PRP to a tendon in my forearm. He is kind- patient. and excellent at performing this latest procedure.

I have seen Dr. Evangelista several times for back issues over the years. During every visit, he takes the time to listen to everything I have to say, and asks lots of questions to thoroughly understand my issue. He also clearly explains scans and X-Ray results so that I can easily understand what the problem is, what needs to be done to treat the issue, as well as prevent it from happening again. I have complete trust in Dr. Evangelista and would recommend him to anyone that needs a physician that will actually take the time to listen to your issue as well as care about your long term health.

About five years ago, I had a major tear of my upper hamstring while playing softball. My regular doctor recommended physical therapy and the physical therapist said it would probably take months to get me back to where I could run and play like I used to. I called Dr. Evangelista and he suggested something that was relatively new at the time, a PRP (Plasma Rich Platelets) injection. It worked like a charm. In less than three weeks I was running with no pain and have had no issues with my hamstring since. I highly recommend Dr. Evangelista if you are trying to avoid a major surgery, as you may find out that major surgery is not necessary. Any sports Related injury, Dr. Evangelista should be your first call. He It's easy to speak to and communicates in terms that are easy to understand for those with no medical experience. The office staff is very friendly and efficient.

I have been a patient of Dr Evangelista for quite some time. As an avid surfer you sustain injuries over the years and Dr Evangelista has always been caring, thoughtful, and extremely thorough. His staff is A+ as well.

I saw Dr. Evangelista after my hip fracture surgery. I went to him for a second opinion after seeing my Dr. I was extremely impressed with his thorough evaluation. He answered all my questions and was very patient and informative. I feel I had very valuable advice. Highly recommend Dr. Evangelista!

I saw Dr. Evangelista for a number of years for back and neck pain. He would assign me physical therapy and occasional steroids for my pain. As my problems progressed to the point that I was developing radiculopathy, could no longer tolerate physical therapy, and became unable to do ordinary things, he STILL would only prescribe physical therapy. He denied that I could have pinched nerves when I asked. After noticing that my radiologist reports were much more insightful and truthful then what the doctor told me, I sought help elsewhere. I finally found a surgeon who did a tri level ACDF due to three herniated disks and obviously a ton of nerve compression. Quickly after surgery, the pain was gone. I believe this doctor to have left me with severe damage and pain for years until I got proper help.

As a colleague I have sent him dozens of patients over the years. He has always had a very good reputation with those patients and I know that he is professional and very competent. I will continue to refer him patients whenever my patients need a sports medicine physician for their care.

I have know Dr Evangelista for many years. He has an excellent reputation and has been very helpful to my overall sports health. I would recommend him to anyone.

If I could give 0 stars I would. Staff is horrible. They continuously sent no show bills to me when I never scheduled the appointments. They scheduled follow-ups without my consent. I had to call in each time to tell them I wasn’t paying and each time I was told they’d remove them. When I recently called to schedule an appointment, I was told I had a no show balance of $25 yet again! I was so angry. I demanded to speak with Dr. Evangelista. I was told he doesn’t call patients but I made it clear that I wanted to speak with him. He did call me yesterday. He actually went on for a few minutes about how he took off the other no show fees but that he wasn’t going to see me unless I paid this no show fee. I adamantly told him that I never scheduled the appointment and therefore I wasn’t go to just pay a random $25 fee. When I asked him to tell me if he had noted in my file that he told me to follow-up any of the 3 times I saw him, he said he didn’t have that information. Patheti!
