Very dishonest in his documentation. I was reading my medical record today, I came across some serious innacurate information. I was thinking; is this even about me. I just want to say that IF I would have seen this sooner; I would have filed a written complaint against him to the medical board. They would have reviewed my record and seen that I have never complained about my shoulder bothering me. My co. car was totalled. It eventually turned into a Work Comp Case because my employer refused to pay my medical bills even though the person who hit me admitted fault and was ticketed. I injured my shoulder and neck. Ended up having rotator cuff surgery. Jay knew I had a Work Comp Case going on. My mistake for telling him. He wrote in my medical record, "right shoulder impingement. Exacerbation from a prior problem." He knows that is falsifying legal information. He should have been held accountable for lying. Very unprofessional. People go read your medical record. You will be shocked

Worst doctor in the area. I have seen multiple specialists and this one in particular didn't know right from left when it came to muscle and nerve pain.
