Irresponsible documentation from this lady. Failed to confirm with me what turned out to be inaccurate information that will replicate in my health history. This person just copied without confering with the patient if it was correct information. Also, I just didn't like her as a person. I had such a bad feeling from her, I fired her and found a much better and more careful and knowledgeble doctor to treat me. I made a very good decision to avoid her and not allow her to care for me.
