No follow up and no phone number to contact him
I havenot been able to contact Dr Michael Koniuch. He does not have a phone number listed on his profile only an address. My father is living in an assisted living facility. Dr Koniuch’s staff installed a monitoring pad on my father’s bed. We have not been provided with any information on the monitor. I unplugged it over 2 weeks ago. Medicare is being billed $400 a month for monitoring that is not being provided.
