FAILED ME AS A PATIENT
Had surgery 11-14 to remove a cyst from my back. Don't give me any time to be off work so therefore i returned to work on 11-16 only to work 3 hours of a 12 hour shift and went home. My shift is Saturday Sunday and Monday 6pm-6am. I didn't work Sunday 11-17. I tempted to go 11-18 but could only work 1 hour. I didn't have anymore time saved up so i was basically not getting paid. Called Dr Choudhury office for a whole week asking if i could have a doctor notice. Messages were claimed to be sent but never received a call back. Had to call the office the next week and left messages again. In between time i was still tempting to work. Some days i could and some i couldn't. Had my follow up 12-4, gave the Dr FMLA and Accommodation papers. They were incomplete. Dr refused to correct the paperwork but had the nurse to tell me he wasn't going to fix the corrections that Sedgwick said were wrong. Also said I should be healed by now. As i told the nurse this is my body and im still in pain!
