Put incorrect information on my record
This provider put incorrect information on my record when I was having surgery. It was for another person, and I have been trying to let him know that I was the patient not the other person. He refuses to take the incorrect information off, because he does not believe I was the patient. I gave the privacy department at uh the phone number of my PCP so he could verify that I never had those serious illnesses but they have not done anything and he has not taken it off my patient record. I'm afraid that it could impact treatment for any further health issues because of this incorrect information. He responded to the privacy office that he didn't believe he made a mistake. I'm the patient and I know whether or not I had those illnesses. And now I can't get hold of him to get it straightened out.
