Derek has an obvious fetish involving relationships and power dynamics. He uses his power, as a therapist, for his own 'personal gratifications'. Instead of providing quality therapy practices, he focuses on his patient's emotional dependence on him. After he has gotten what he wants from his 'patient' he will discard them like garbage. Meanwhile, assuring they are too afraid of public humiliation or other consequences to come forward and report him for his violations. It is just a matter of time before another one of his patients is involved in some sort of self harm or at the very least, uneeded emotional trauma.

Derek has a history of erotic-counter transference. He has used his power of influence, as a therapist, to romantically seduce a married woman, who was suffering from mental health issues that left her extremely vulnerable to his advances. Derek is also married and has 3 small children. His public appearance as a 'sweet Christian, family man' is a facade. The only reason that he has not been reported to the licensing board, is that he uses guilt and fear to manipulate his patients into not reporting him.
