Poor patient care
I have been coming to Dr. VanDyke for WC since 2022 & have not had any resolution for my injuries! I have attended appointments just to sit a conversate with the doctor, I was taken off work on 08-14-2024 & I was not instructed on how to apply for any form of EDD or SDI by any of the staff until WC payments kicked in. The staff because they assumed I knew the process being I was coming to the office for awhile now, & this assumption has caused a great financial hardship! I've requested to have Aflac forms completed for that time off given by the Dr, & have been denied that until I come in a visit & pay $40 for the form, also Dr VanDyke is requesting documents from my HR department when her office is the one whom entered the dates on their form for me to give to my employer. This is unfair treatment to me! No one has compassion or empathy. I guess this is the only way to make extra money is to charge WC for a visit to complete a form, not to discuss my injury.
