Dr. Berman and I sat in his office and he explained to me that he would write a summary statement instead of filling out my disability paperwork. About a week later, I called Dr. Berman and asked if he had prepared the summary statement. He said he did not know what I was talking about and that he didn't recall saying he would prepare any such summary report. He denied knowing anything about it several times until I reminded him of our meeting in his office. He finally admitted what he had done
