Dr. Forman spends adequate time with me and provides a thorough exam. Very easy to discuss my concerns with him. I trust that Dr Forman is doing the best possible eye care for me. Office personnel have always been courteous and friendly at his Carmel office. Eric Koehler

Dr. Forman and his staff make you feel like you’re family. He is an excellent doctor. Extremely thorough exam. Actually talks to you, spends time, listens and ask you the right questions to get you exactly what you need. Office is clean and convenient w/ plenty of parking. Jennie is truly a pleasure and a big component in making that office run so smoothly. I highly recommend Dr. Forman for an absolutely perfect experience!

I have been a patient of Dr. Forman's for years. I have s specialized eye issue that only he was able to figure out. He was able to explain the condition thoroughly. He is thorough, patient and caring. I wouldn't go anywhere else!

Dr. Forman was very patient, took his time explaining everything to me, and did a thorough examination. I felt I was in good hands -- his demeanor and professional knowledge both helped make my appointment a positive experience. I would be inclined to recommend Dr. Forman to any friend or colleague seeking high-level optometric service.

I made an appt. for a routine exam. No one told me that he wanted to dilate my pupils. I said " I can't, I'm driving". He says make another appt. So I do and my insurance won't pay the 440.00 he wants for this. Technically, it was 1 appt worth of exam. I tried to fight it, to no avail. Picking out frames, the woman says "this scrip is expired". He wrote 2015, instead of 2017!!!! So now Ihave to wait for them to send a new one and spend another day off trying to get glasses. Completely inept.

In general, I'm not the best patient and I get very anxious any time I have to go for an appointment. But I must say Dr. Forman excelled at putting me at ease, performing the necessary checkups in a comforting and supportive manner and offering clear and thorough explanations of what he was doing and what the results meant. His recommendations and advice were very helpful. I left his office feeling I got the best professional care I could have hoped for.

The office does not accept our family health insurance so they refused to make an appointment for my son even though we've been patients of MKMG/Caremount for many years AND I was more than willing to pay out of pocket. They recommended I change the insurance to one that Dr. Forman takes... This is a first I've heard of such a policy. In the end, I made an appointment with a different optometrist who also doesn't accept our insurance but will take payment via credit card/check/cash.

I am so grateful to Dr. Forman. At one point I was seeing three different doctors. One for regular exams, one for an eye infection and then a third to fill prescriptions for contacts and glasses. Until I discovered that Dr. Forman was excellent in all three. He genuinely cares about your eye health and providing the best vision possible. He takes his time allowing you to decide which option is better. Jeannie in his office is truly the warmest and efficient person. They treat you like family.

Improper eye dilation of a 12 year old girl that led to 2 additional dilations and weeks of waiting. Another doctor in the same practice said that his treatment of my daughter was not correct. Oh, and he refuses to answer my repeated mailings.
