The office has this scheme where they don’t answer the phone then charge you $85.00 just to cancel appointment and they are very rude about it!!! Something needs to change . And the Dr is just a pencil pusher doesn’t really care about the patients just make his money off of prescription of shot and pills very bad practice!!!!!!

Never seen on time. Usually waited around 45 minutes. When finally seen I walked in, the doctor barely spoke, and dismissed me. On top of that they are trying to charge me erroneous charges.

The office and this doctor do not listen to patients. I was never told a Rx I had they would not fill, another Rx was never renewed even after multiple calls to the office. My Rx’s that were filled were sent to the wrong pharmacy and took a couple days to fix it. This doctor even sent me a text stating they think they sent my Rx to the wrong pharmacy. When my Rx were filled and ready one was an Rx I had never taken nor knew anything about so I did not pick it up. Emailed the office about it hoping I may get a response and was told that was a mistake and should have been pulled. Final straw was the doctor calling almost an hour late for an appointment and left a message to call and a text to me. I have the times recorded as to what time he called and texted me for the appointment and he said he called at the actual appointment time. The practice does not listen to anything you say. Initial appt was barely five minutes and the co pay was $45. Recommend avoiding the office

I would recommend The Office for a Healthy Mind and Wellbeing to anyone looking for great doctors and staff especially the ladies in the front office who care about their patients and the whole staff are very professional and friendly.

This practice requires your social security number(SSN) in order to be seen even with valid primary, secondary insurances and a State issued ID. It is common knowledge in the medical field that insurance companies use member ID numbers instead of SSNs to assist in protection against identity theft. Yet, this practice still requires it. I'm just saying, according to Ginger, staff member? nurse?, who gave us no explanation except making it perfectly clear, "This is a private practice, SSN

Awful. No help. Overloaded and pitiful. Don’t bother!

I see Dr. De Carvalho and have seen his ARNP Brian Beardsley as well both are very warm and kind. They want to help their patients and are very concerned of their wellbeing and want the best for them. I do not understand some of the negativity I have read I think it is because people want to tell the doctor what medication THEY WANT and do not want to listen to what the doctor is trying to explain what is best for them medically and some medications are NOT THE BEST! I love this office and staff

I haven't met Dr. Decarvalho but the office staff in orange park has been underwhelming. No one returns messages, I consistently have to follow up with them to ensure they are completing tasks as they pertain to me, and Nathalie has yet to return a VM I left regarding my unfilled prescription nearly 2 weeks ago. My PCP referred me because of Dr Decarvalho but I'm looking into other providers because the lack of care is unacceptable.

I love everything about both of his offices and staff! He has changed my life around he genuinely cares about the well being of his patients and wants them happy and healthy. His Nurse Practioners are very good to I have seen 2 of them and they are very attentive. The staff is wonderful I have seen people yell and them and not be nice and they still smile and try to keep everyone happy.

This office is welcoming until you refuse to go to one of their two detox centers that Dr. Decarvalho is part owner in. Med compliant for years, and in first visit they want to change me when I've been completely stable. They are all about money an billing patients they won't see who won't buy into their facility process. Patient can be managed outpatient when stable and know who the office is and where their financial stock lie.

Dr. Decarvalo has helped to get me stable with the medications he has me on. He reduced many of the medications my previous doctor had me on, and I'm not as lethargic as I had been.

Dr De Carvalho and his office are wonderful. They saved my life! The staff is very warm and welcoming, friendly. I have no problem waiting to see a GOOD DOCTOR! Sometimes the wait is long but he is such a breeze to talk to and went over my medications. I am not joking he literally saved my life with all the changes I had to be patient with him and he did all he could to help me. He took so much time with me that is why there is a wait he does not force people out of his office.
